The Clinical Review of Acute Cholecystitis

Abstract
A clinical review was performed ov the 216 cases. of acute cholecystitis who had been admitted to the Department of Gastroenterology, College of Medicine, Korea University from January 1971 to October 1977.
The results obtained were summarized as follows:
1) Of the 46966 patients (from Jan. 1971 to Oct. 1977 216 patient (0.46%) were acute cholecystitis.
2) In these cases, there were 93 males (43.06%) and females(56.94%), a ratio of 1 to 1.3. The average age of these cases was 46.4 years.
The youngest patient was 11 years of age.
The oldest was 83 years of age of the 216 patients 142 (65.74%) were 31 and 60-year of age.
The highest monthly incidence of acute cholecystitis in this report was between September and October.
3) Epigastic discomfoi t was the most common prodromal symptom, being present in 60.5% of the these cases. But R.U.Q. pain was the predominant
clinical symptom; being present in 81.9% of these cases and the biliariccolic were 124 patients (57.7%) with radiating pain to the rt. shoulder site, 55 cases (25.6%). Indigestion, fever, vomiting, nausea, jaundice, chillness, anorexia, diarrhea, constipation, itching sensation, were the other clinical symptoms in the order of, frequency in our cases.
4) Tenderness on the R.U.Q. was the frequent physical finding, being present in 72.1 % of these uses.
The percentage of palpable mass (10.7%) was lower than the foreign country, the recognized jaundice (29.8%) was higher the foreigner which the reason probably due to more common biliary stone ¢¥of common bile duct duct than the foreign country.
5) The average W.B.C. count was 17, 337/mm3 (shift to left) and the number of total count more -than normal range was in 60.8 percentage in this report.
The number of serum cholesterol level more than normal range was in 47% and also the serum alkaline phosphatase over the normal range was in 65.2 % in ¢¥these cases. The reason may be biliary tract obsruction by common bile duct stone and inflammation ,of biliary tract.
6) The biliary stone visualized in 3.03% by simple abdomen, 17.82% by IN. cholangiogram, 14.26% by Oral cholecystography in these cases.
There were 6 gall bladder stone (27.3%) and 14 ,,common bile duct stone (56.52%), a ratio of 1 to ~.1.
7) The incidence of. complication was in 12.96 percentage and the most predominant complication was pancreatitis in these cases.
Medical treatment was done in the 159 cases(73.6 6) and surgical operation performed in the 51 cases (23.696)., 2 cases died after surgical operation.
The one died of complicating pancreatitis and the other died of bile peritonitis due to perforation of gall bladder.
